11. Treatment of related gynaecological problems

There are many gynaecological disorders that can contribute to your infertility. The following is a brief account of some common disorders.

Endometriosis

In this condition, normal womb lining is found in sites outside the womb like the ovaries and the rest of the pelvis. The cause of this condition is unknown. Infertility can occur even if the fallopian tubes are patent.  It affects females in the reproductive age group. Typical symptoms include period pains, painful coitus and irregular periods. Treatment is required for relief of symptoms and/or infertility. This may involve different medications or surgery. Where natural conception does not occur with treatment, assisted conception may be necessary.

Pelvic infection

Pelvic infection can lead to tubal damage and blockage. The common infections are gonorrhœa and chlamydia. These infections may go unnoticed. In particular, chlamydia infection is increasingly common. The treatment is the administration of appropriate antibiotics. However tubal damage may be irreversible. IVF is particularly useful for tubal blockage.


Polycystic ovarian syndrome (PCOS)


It is a complex hormonal disorder characterized usually by obesity, hirsutism, scanty or infrequent periods, insulin resistance and infertility. Multiple small cysts develop in the ovaries. Fertility drugs can be used to overcome the problem of non-ovulation. However in resistant cases, surgical ovarian drilling may be necessary. Recent research has linked this condition to high insulin levels and the anti diabetic drug, metformin is used to treat PCOS. Assisted reproduction may be necessary when all treatment fails.

Fibroids

These are very common muscular growths of the womb. They are rarely cancerous. Often they do not cause any problem. Sometimes they cause menstrual flow disturbances and distortion of the womb cavity. These may interfere with conception.

Malignancies

Although malignancies are not directly related to infertility, its treatment can often lead to it. In these instances, fertility sparing operations, cryopreservation of oocytes, embryos and sperms offer the chance of future child bearing.

Pregnancy

When you become pregnant, it is important to see your obstetrician early to ascertain the site, number and viability of your pregnancy.

There is no evidence to show that babies conceived through assisted reproduction are at a greater risk of fetal abnormalities. However, we encourage all our pregnant patients to have at least two dedicated ultrasound scans during their pregnancies:

  • 11-13 weeks scan for Nuchal Translucency (NT) and visualisation of Nasal Bone (NB) to determine Down’s syndrome risk

  • 18-22 weeks scan to exclude structural fetal abnormalities.

Sime Darby Medical Centre Subang Jaya has an in-house cytogenetic laboratory that carries out chromosome analysis. Chorionic villus sampling and amniocentesis for chromosome analysis on mothers who are at risk are routinely carried out. 

Close supervision is recommended for all pregnancies conceived through assisted reproduction. There is a higher risk of multiple pregnancy, miscarriage, bleeding problems, fetal growth problems and premature delivery. It is strongly recommended that delivery be in a hospital that has neonatal intensive care facilities.
